Prophylactic pacemaker use to allow beta-blocker therapy in patients with chronic heart failure with bradycardia

چکیده

Health technology The study compared two treatment strategies for patients with chronic heart failure (CHF). The strategies compared were the conventional strategy and the pacemaker-carvedilol strategy. In the conventional strategy, patients were treated with angiotensin-converting enzyme inhibitors, diuretics and digoxin. In the pacemaker-carvedilol strategy, patients received conventional treatment therapy and had a permanent pacemaker implanted to maintain adequate heart rate after the initiation of beta-blocker therapy with carvedilol. Pacemaker therapy consisted of the implantation of a dual-chamber pacemaker set to an atrial-based rate-response pacing mode (DDDR mode with long atrioventricular (AV) delay).

Prophylactic pacemaker use to allow h-blocker therapy in patients with chronic heart failure with bradycardia

Background Although the benefits of h-blocker therapy for patients with congestive heart failure (CHF) are independent of pretreatment heart rate, patients with chronic systolic heart failure and low resting heart rates are often excluded from h-blocker therapy. We investigated the effectiveness and cost-effectiveness of prophylactic pacemaker insertion to facilitate h-blocker use in these pati...

How many patients will benefit from a new therapy for chronic systolic heart failure, and what difference does it make?

T he importance of heart rate lowering in optimizing natural history among patients with chronic systolic heart failure (HF) has been increasingly clear for 15 years, certainly at least since Kjekshus and Gullestad reported a cross-sectional analysis of the HF mortality trials then completed (1). This assessment revealed that placebo-subtracted mortality changes among the trials were directly p...

Effect of beta-blocker treatment in dilated cardiomyopathy with bradyarrhythmias.

This study was performed to evaluate whether beta-blocker therapy was effective in patients with nonischemic dilated cardiomyopathy (DCM) and bradyarrhythmias supported by pacemaker implantation. Beta-blocker therapy is useful for some patients with DCM, especially those with rapid heart rate or residual nonfibrotic myocardium in the left ventricle, but no data exist on whether beta-blocker the...

Ivabradine Versus Beta-Blockers in Patients with Conduction Abnormalities or Left Ventricular Dysfunction Undergoing Coronary Artery Bypass Grafting

Postoperative rhythm disorders are a serious complication of coronary surgery and they are associated with increased morbidity and mortality. Atrial fibrillation is the most common complication after cardiac surgery, with an incidence of 30% after coronary artery bypass grafting (Camm et al.,2010). There are few data about the etiology of atrial fibrillation in this setting, factors such as int...
